Disability Rights North Carolina releases the results of their investigation of suspicious deaths linked to inadequate discharge planning from the state's psychiatric hospitals. The report contains Disability Rights NC's conclusion that the deaths of at least three former patients in a six-month period resulted from inadequate discharge planning from state psychiatric hospitals. Disability Rights NC, the State's protection and advocacy system, will offer several recommendations to improve discharge planning. The state has statutory responsibility to ensure that patients at state-operated facilities receive appropriate discharge planning.
